OSHA 300 and 300A forms
As we enter 2023, we at AOL wanted to remind everyone of the posting requirements for the OSHA 300 and 300A forms. You can find instructions and forms at Oregon OSHA and through Federal OSHA by following the links provided or visiting their websites.
OSHA 300, Log of work-related injuries and illnesses
First, let’s take a look at the300 log . This form is a record of all reported on the job injuries that occur during the calendar year. The 300 log lists individual names that were injured, date of injury, short description of the incident, days away from work, days on restriction (light duty), and to choose one of the six injury classes (injury, skin disorder, respiratory condition, poisoning, hearing loss, all other illnesses). .
What is considered an injury or illness for the 300 log?
An injury or illness is work-related if an event or exposure in the work environment caused or contributed to the condition or significantly aggravated a pre-existing condition. Work-relatedness is presumed for injuries and illnesses resulting from events or exposures occurring in the workplace. The work environment includes the establishment and other locations where one or more employees are working or are present as a condition of their employment.
Record work-related injuries and illnesses resulting in one the following:
* Death * Loss of consciousness * Days away from work * Restricted work activity or job transfer * Medical treatment beyond first aid (going to the doctor, ER, urgent care) OSHA 300A, Summary of work-related injuries and illnesses
OSHA form 300A is basically a summary of the information provided on the OSHA 300 form, with a couple of additions. The 300A lists the total for each of the categories listed in the 300 form, days away from work, and days with work restrictions or transfers (light duty). You will need to list your company location, industry description (forest activities, logging, trucking), the North American Industry Classification System NAICS codes, average number of employees, and total hours worked by all employees.
Here are the common North American Industry Classification System codes (NAICS) for our industry – * 113310 (Logging) * 115310 (Support activities for ag & forestry) i.e. reforest, firefight, PCT thin * 484220 (Specialized freight trucking) i.e. log truck, dump truck, tanker truck, flatbed truck * 237900 (Other heavy and civil engineering construction) i.e. road construction * 811310 (Commercial and industrial machinery and equipment repair, except not automotive and electronic) * 811100 (Automotive/truck repair) * 561730 (Arborist services) * 321113 (Sawmills) * 321210 (Plywood mills) Posting Requirements
One common misconception is that the 300 log needs to be posted. In fact, the 300A Summary of work-related injuries and illnesses is the form that is required for posting. The 300A needs to be posted from February 1 to April 30th. Be sure to keep both the 300 and 300A forms for your company records.
