As we enter 2024, AOL wants to remind everyone of requirements for the OSHA 300 and 300A forms. You can find instructions and forms at Oregon OSHA and through Federal OSHA -- by following the links provided or visiting their websites. You will need to annually complete forms 300 and 300A, to record work-related injuries and illnesses that resulted in any of the following:
Death
Loss of consciousness
Days away from work
Restricted work activity or job transfer
Medical treatment beyond first aid (going to doctor, ER, Urgent Care)
One common misconception is that the 300 form needs to be posted. In fact, the 300A Summary of work-related injuries and illnesses is the form required for posting. Be sure to keep both the 300 and 300A forms in your company records.
300A form must be posted from February 1 to April 30th.
